Case-Based Panel Discussion: Non-Muscle Invasive Bladder Cancer – Summary:

Seth P. Lerner, MD, Guilherme Godoy, MD, MS, and Jennifer M. Taylor, MD, MPH, take part in a panel discussion consisting of two case studies of non-muscle invasive bladder cancer, focusing on BCG usage for high-risk patients and immediate intravesical post-operative chemotherapy instillation for medium-risk patients.

Abstract:

In the first case presented in this panel discussion, Dr. Taylor discusses a 73-year-old male patient with a high-risk pathology profile who underwent conventional BCG treatment that failed to prevent a recurrence of disease. Dr. Lerner details the reasons for the shortage of BCG and the benefits of early radical cystectomy on patient survival, as well as alternative treatments to radical cystectomy and FDA guidelines for trials of these treatments.

In the second case, Dr. Taylor presents a 76-year-old female patient with a medium-risk profile who was not a candidate for BCG treatment due to the aforementioned shortage. Dr. Godoy outlines alternative treatment options, focusing on immediate intravesical post-operative chemotherapy instillation as a well-tested treatment to decrease the risk of recurrence (rather than progression) of disease in these patients. Additionally, he explains why this treatment is currently under-utilized.

Innovations in Urologic Practice (Innovations) is an annual, multi-day, CME-accredited conference devoted to innovative diagnostic and treatment strategies for and controversies related to some of the most common urologic problems in the current era. The topics covered include oncological management of the bladder, kidney, and prostate. Jennifer M. Taylor, MD, MPH, is an associate professor of urology and a practicing urologist at Baylor College of Medicine in Houston, Texas where she also serves as the residency program director for urology. Additionally, Dr. Taylor practices at the Michael E. DeBakey VA Medical Center where she is the director of urologic oncology for the VA urology service. Dr. Taylor earned her MD from the University of Texas Medical School in Houston, where she also completed an internship and residency, and she completed a fellowship in urologic oncology at Memorial Sloan Kettering Cancer Center in New York City. She also earned an MPH from the Harvard T.H. Chan School of Public Health in Boston.

Dr. Taylor is involved in clinical research in multiple genitourinary malignancies, with a focus on bladder cancer, as well as in research on medical education and physician wellness and resilience. Dr. Taylor is involved in the national medical community and is a member of the American Urological Association, the Society of Urologic Oncology, the Society of Women in Urology, and the American College of Surgeons. She has won numerous awards, including Baylor College of Medicine’s Norton Rose Fulbright Faculty Excellence Award for Teaching and Evaluation and the Women of Excellence Award.
