Optilume® BPH Catheter System: Optilume® Care Pathway and Treatment Options

E. David Crawford, MD, Stephen Kaplan, MD, and Kevin T. McVary, MD, FACS, explore the utility of the Optilume® BPH Catheter System, a novel minimally-invasive surgical therapy (MIST). This is the second in a series supported by Laborie.

Dr. McVary begins with an overview of the current BPH market, diagnostic, and treatment algorithms and pharmaceutical/surgical treatment pathways. He illustrates the growing population of men with BPH who are underserved by pharmaceuticals, and those who are concerned about long-term effects of medication on their sexual function. Additionally, he discusses the dangers of delaying surgical management when it is indicated.

Dr. Kaplan then compares the function and outcomes of the Optilume® BPH Catheter System to those of current MIST and invasive surgeries. With an emphasis on the preservation of sexual function and low intraoperative risk, Dr. Kaplan and Dr. McVary discuss the clinical data, their experience with using the Optilume® BPH Catheter System, and their expectations for real-world data.

Optilume® BPH Catheter System: Optilume® BPH Product and Clinical Data Overview

E. David Crawford, MD, sits down with Dean S. Elterman, MD, MSC, FRCSC, and Stephen Kaplan, MD, to discuss the evolution of BPH treatment and the Optilume® BPH Catheter System, a novel minimally-invasive surgical therapy (MIST). This is the first in a series supported by Laborie.

Dr. Elterman begins with an overview of the Optilume® BPH Drug Coated Balloon Catheter. He presents the components of the system and their utility. He presents a demonstration of the system in action.

Dr. Kaplan presents data from the clinical investigations of the effectiveness of the Optilume® BPH Catheter System. He provides an overview of the results from the EVEREST study and the landmark PINNACLE trial, which was published in the September 2023 issue of the Journal of Urology. He notes that the major benefits of the treatment include significant flow rate improvement despite the lack of tissue removal and low surgical retreatment rates.

Focal Cryoablation

Michael A. Gorin, MD, discusses the value of focal cryoablation as a treatment option for prostate cancer in modern practice. He begins by providing a brief overview of negative patient outcomes after whole-gland treatment, and the benefits of subtotal gland treatments in the form of focal ablation therapies like cryoablation and High Intensity Focused Ultrasound (HIFU) therapy.

Dr. Gorin notes that HIFU and cryoablation are the two most commonly used and studied modalities for prostate cancer treatment. He discusses the benefits and weaknesses of focal HIFU, with particular focus on the possibility of incomplete cancer treatment, and compares them to those of focal cryoablation.

Dr. Gorin presents the elements of focal cryoablation which reduce the risk of incomplete treatment. He presents current guidelines and devices for performing focal cryoablation which protect the patient against side effects, like urethral sloughing, which had been previously associated with cryotherapy.

Dr. Gorin concludes by demonstrating the long-term success rate for patients treated using focal cryoablation. He compares the QOL outcomes of focal cryoablation to those of HIFU, and presents a recording of a real focal cryoablation procedure.
