The Role of Local Hormone Therapy for Genitourinary Symptoms of Menopause – Summary
Katherine Amin, MD, explores the role of local hormone therapy in managing genitourinary syndrome of menopause (GSM), emphasizing its effectiveness and safety. In this 19-minute presentation, Dr. Amin outlines the physiological changes in the vaginal and urinary epithelium due to estrogen loss, which disrupts the microbiome and leads to increased pH and inflammation.
Dr. Amin highlights vaginal estrogen therapy as the most effective treatment, improving symptoms within 12 weeks without increasing the risk of breast or endometrial cancer. The discussion includes various formulations such as creams, rings, and suppositories, with evidence supporting their long-term use. Non-hormonal options, including lubricants, hyaluronic acid, and pelvic floor therapy, are also considered for specific patient populations.
Dr. Amin emphasizes physician-driven education, as many women remain unaware that GSM is treatable. Clinicians are encouraged to actively screen for symptoms, engage in shared decision-making, and reassure patients about the safety and efficacy of treatment to enhance quality of life and prevent complications.
