In conversation with Peter K.F. Chiu, MD, PhD, FRCSEd, Honorary Clinical Assistant Professor and Associate Consultant in the Division of Urology at the Chinese University of Hong Kong, Yao Zhu, MD, PhD, attending urologist at Fudan University Shanghai Cancer Center and Associate Professor in Oncology at Shanghai Medical College, discusses the increasing number of deaths from prostate cancer in East Asia, emphasizing the need to develop Asian-specific genetic risk models. Dr. Zhu notes that while data on non-Western populations has been relatively limited to this point, there appear to be numerous differences in prostate cancer-related mutations between East Asian men and Western men, including lower rates of TMPRSS2-ERG fusion and PTEN deletion, and increased prevalence of FOXA1, SPOP, CHD1, and BRCA2. He also observes that there are pharmacogenomic implications to differences in testosterone metabolism-related genes between East Asian and Western men.

Challenging Cases in Advanced Prostate Cancer: nmCRPC

R. Jonathan Henderson, MD, a urologist with Regional Urology, LLC, discusses an unusual case of non-metastatic castration-resistant prostate cancer (nmCRPC) with a panel featuring Laurence Klotz, MD, FRCSC, Daisaku Hirano, MD, and Michael S. Cookson, MD. The case in question involves an asymptomatic patient in his 70s who was successfully treated for localized prostate cancer and then, ten years later, was treated with cryotherapy for recurrent cancer, after which point cancer stopped appearing in imaging, but the patient’s PSA rapidly rose even as he was treated with abiraterone. Drs. Klotz, Hirano, and Cookson discuss whether it is appropriate to give such treatments to a persistently asymptomatic patient and note that his high PSA could be a response to abiraterone. They also consider the possibility that advanced imaging techniques like PSMA-PET would reveal that the man does have metastases and ponder which of the approved treatments for nmCRPC might suit this patient best.

MRI-Guided Transurethral Ultrasound Ablation (TULSA) in Patients with Localized Prostate Cancer

Laurence Klotz, MD, Professor of Surgery at the University of Toronto and Sunnybrook Chair of Prostate Cancer Research, discusses a newly FDA-approved treatment of localized prostate cancer: MRI-guided transurethral ultrasound ablation (TULSA). He explains how using MRI improves ablation technique, offers a technical overview of the procedure, and then reviews clinical trials of TULSA. As Dr. Klotz describes, MRI thermometry allows physicians to see what temperature the tissue is reaching during ablation, and the use of urethral and rectal cooling helps protect the urethra, making TULSA an effective treatment. He then explores a clinical study of MRI-guided TULSA conducted in five countries that showed a significant reduction of the prostate with minimal adverse effects.
