APP-Directed Management of Female Sexual Health and GU Syndrome of Menopause – Summary
Aleece R. Fosnight, MSPAS, PA-C, CSCS, CSE, IF, NCMP, HAES, discusses APP-lead treatment of female sexual health and genitourinary syndrome of menopause (GSM). Ms. Fosnight begins by describing symptoms of GSM, including hormonal, sexual, and GI symptoms. She notes that while up to 84% of menopausal women face GSM symptoms, half have never been treated.
Ms. Fosnight then turns to GSM diagnosis and treatment options, including hormonal treatments, non-hormonal treatments, and physical therapy. For each treatment, she addresses safety, side effects, and contraindications.
Ms. Fosnight concludes by emphasizing the importance of clear communication and preventative action. As many menopausal patients are likely to experience GSM, she emphasizes the importance of a frank, supportive dialogue between APPs and patients, noting that there are many safe treatment options available.

ABOUT THE AUTHOR
Aleece R. Fosnight, MSPAS, PA-C, CSCS, CSE, IF, NCMP, HAES, (She/Her/Hers) is the founder, owner, and director of the Fosnight Center for Sexual Health. She is a board-certified physician assistant specializing in sexual medicine. She earned her Master of Science in physician assistant studies from the University of Kentucky in 2011 and completed a
postgraduate certificate in sexual health counseling and education from the University of
Michigan Sexual Health Certificate Program in 2014.
Ms. Fosnight is a nationally certified sex counselor/supervisor and educator through the American Association of Sexuality Educators, Counselors and Therapists (AASECT). She is an LGBTQIA+, BDSM/kink-aware, trauma-informed professional, and a Health at Every Size (HAES®) provider. As a North American Menopause Society (NAMS) Menopause Certified Practitioner, Ms. Fosnight also evaluates and treats women through the menopause transition. Her expertise includes pelvic floor dysfunction, urinary incontinence, pelvic organ prolapse, pelvic pain, low libido in men and women, interstitial cystitis, menopause transition, effects of cancer and cancer treatments on sexuality, pregnancy/postpartum sexuality, female sexual health, transgender medicine, low testosterone, Peyronie’s disease, and erectile difficulties including erectile dysfunction, premature ejaculation, and delayed ejaculation.
Ms. Fosnight routinely presents at local, state, and national organizations. She is currently the chair of the Sexual Medicine Society of North America (SMSNA) Advanced Practice Provider (APP) Committee, the chair of the International Society for the Study of Women’s Sexual Health (ISSWSH) Membership Outreach Committee, and the chair of the North Carolina Academy of Physician Assistants (NCAPA) Regional Chapters Committee.
